Spinal cord injury (SCI) is the injury of the spinal cord from the foramen magnum to the cauda equina which occurs as a result of compulsion, incision or contusion. The most common causes of SCI in the world are traffic accidents, gunshot injuries, knife injuries, falls and sports injuries

SCIs are characterised as traumatic or non-traumatic events that result in neurological damage affecting motor, sensory, and respiratory functions, as well as bladder, bowel, and sexual function. The individual’s blood pressure, skin integrity, and ability to control temperature are all affected by the neurological disruption.

Five key steps in the management of individuals with SCI are

  • Assessing impairments, activity limitations and participation restrictions
  • Setting goals relevant to activity limitations and participation restrictions
  • Identifying key impairments that are limiting achievement of goals
  • Identifying and administering physiotherapy treatments (strengthening, joint mobility, motor skill development, cardiovascular fitness, respiratory functioning, pain managing)
  • Measuring the outcome of treatments

All physiotherapists should be aware of the following concepts and how they apply to the management process.

  • Injuries to the vertebrae (acute phase – affects the intensity of therapy)
  • Shock to the spine (reflexes are temporarily absent for 3 days to 3 months)
  • Autonomic Dysreflexia (SCI above T6 – causes sympathetic activity to rise)
  • Orthostatic Hypotension is a condition in which the blood pressure (blood pressure associated dizziness due to postural changes – mobilising from supine to sitting)
  • Deep venous thrombosis (DVTs) and pulmonary embolus (PE) are two types of thrombosis (know the signs and symptoms of these conditions to ensure safe therapy)
  • Dysfunctions of the Bladder and Bowel
  • Ulcers caused by pressure (prevention forms part of therapy sessions)
  • Heterotopic Ossification is a type of heterotopic ossification that occurs when (calcification of soft tissue that can lead to contractures)
  • Psychological and Emotional Distress (identify signs and symptoms and recommend to team members who can help).
